Medicare

In early 2022, the Centers for Medicare and Medicaid Service (CMS) announced they were developing a new model of care for Medicare fee-for-service patients to help improve patient outcomes and address underserved communities. The ACO Realizing Equity, Access, and Community Health (REACH) model would build upon the previous successes of the Global and Professional Direct Contacting (GPDC) model and the Medicare Shared Savings Program (MSSP). This new ACO REACH structure would offer more efficient care coordination for beneficiaries and higher risks and rewards for participating providers. The goal is to lower the cost of care by improving patient’s health equity and provide a higher quality of services to underrepresented populations.

Leading up to the January 2023 launch date, SMG has worked closely with our ACO REACH partners at Medical Home Network (MHN) to successfully develop and implement extensive claims processing workflows, customer service platforms, care coordination payments, voluntary alignment procedures, provider correspondence, CMS reporting requirements, and beneficiary outreach across multiple regions and communities. We are proud to offer our services to this new Medicare ACO REACH model to achieve the highest standards in health care administration. 

In addition to extending our services to Medicare REACH ACO’s, we are also a verified Business Partner of the National Association of ACO’s. With this partnership, we are committed to helping Medicare, Medicaid, and Commercial Insurance ACO’s succeed in their efforts to reduce health care costs while providing patients with the highest quality value-based care. We support the NAACO in their mission to work with CMS and State and Federal governments to improve performance and grow the number of individuals in quality accountable care relationships. We will be in attendance at both their Spring and Fall 2023 Conferences, and we look forward to collaborating with their over 400 member ACO’s to improve population health, enhance patient experience, reduce costs, and advance health care quality and equity.
